The Jac-Kof Tool is supplied complete with hardware. The kit also includes an axle strap and wheel strap.

I have one, too. It's of somewhat limited use to me since it doesn't work as well with my ARB bumper as it does on the 80 series ARB. Plus I have a Marlin rear bumper and square sliders. The 4Runner and Pickup ARB is narrower and the land for the tool doesn't sit flush. On the 80 series ARB and the Slee rear bumper and sliders, the thing is the bee's knees. It's well made and does what it's advertised to do.

How wide of a clevis mount will fit in the Jac-Kof? The one on his site does not look that wide? Mine are 1" wide.

I just use a clevis in a clevis hole. Put the clevis' loop over the lifting tongue of the Hi-Lift. Works a charm and still allows you to push the truck off the jack sideways if that is what you need to do to get unstuck.
