Here's a map that shows, county by county, where 3G cell phone service is available in USA. The map's purpose is to identify counties eligible for proposed government funding. But the map is also useful to see where you might have problems with cell phone reception when traveling off the beaten path.
The map has a +/- control in the upper left to zoom in and out. The map can be dragged around with your mouse to reveal the area you're interested in.
Note that the map is talking about 3G data service, so you may be able to make a cell phone voice call in the areas marked as having no service.
Looked at another way, the areas marked in black are good candidates for "off the grid" adventures!
