I'd love to widen my topper on my tundra a bit so I could put the bed platform on the cab rails and sleep sideways. That would open up a few feet in the back of the bed for storage and moving around.
Got the idea from looking at the sidepods/flares they add to the sides of sprinter vans to be able to sleep sideways. They stick out 6" or so on each side. You can buy the molded flares, would then need to cut the sides of a topper and fiberglass them in.
