What options in the U.S.?

What options are there in the U.S. for a 4 x 4 cab and chassis? And what is the correct term for this body configuration. Sitting forward over the engine?

Cab Over Engine - COE, or Forward Control.

FUSO makes the FG, and that's about the only medium duty FC chassis out there, that's factory 4x4. If you want to go heavier, you can get military surplus, and specialized commercial trucks that are 4x4, but not too common.

Everything is a compromise.

Or a custom build like I am doing. Adding a transfer case and front axle to an existing 2wd NPR is doable.
 
Are there any places in So-Cal that will convert an isuzu to 4x4 that folks would recommend? I'm so hung up on the single speed t-case and lack of real auto in the FG. Would love to be able to cruise reasonably on the highway and also have a low range...

Pac Fab in So Cal can do just about anything, along with Penhall, Brenthel, Geiser and a bunch of other very capable custom fab shops. The same goes true for Arizona and Vegas where there are plenty of people with UTV's, Trophy trucks, Class One, Jeep Speed, etc.

Southern California is THE place to be for custom fab work, you have your pick of shops. How deep are your pockets?
