I did that several times on my truck camper till I came home one day and made a strap for it that held the cap on. I had an old piece of Mechano strap that I bent to hold the cap on, screwed it to the base and never had an issue again.
I was checking out this dealers listings last week as they seem to have a few. The only thing that I noticed with the ambulances that I have looked into in western Canada is that most have REALLY high mileage or the lower mileage ones seem to be priced to capitalize on the medical accessories...

I understand what you are saying but if you are heading out for a week or two without a fridge or the ability to resupply a steak may not fit the no refrigeration requirement. Many times fresh food is not easy to carry.
Did you check the clearance for the fridge and the air flow requirements? Yours looks a little tight in there and if you have it lined with foam you may find it overheating.
