Looking good! Bad alignment sounds like a great excuse to put in a D60 to me! You can do a free tape measure alignment to get your toe correct in about 10 min. If you need one I can make a quick video and post it on youtube if you are not sure how to do one.
Probably lift pump or low oil level. Your injectors are fired off oil pressure from your HPOP and if the oil level is low it wont fire the injectors properly.

Thanks for the kind words! There are few things worse than a worn out TTB leaf sprung front end! Mine had less than one inch of up travel when I removed it and the bushings were so shot they were metal on metal.

I wouldn't worry about your bedsides flexing out at all. Your rack is stronger than your bed and if anything will keep them from bowing not causing it. That is more a problem for guys running Thule and Yakima racks that are not a structural member like your rack is. I bet it doesn't even know...
