Adding two-wheel lift tow capability to ambulance conversion

I have a 6.6 duramax chevy ambulance with pretty beefy support rails near the back. I have been contemplating if it would be possible to rig something to lift my (TINY, light) 2000 Honda insight off of its front wheels and onto a support attached to the back.

The idea is that I want to avoid a tow dolly and instead simply lift the front end by the wheels like a "wheel lift" tow style thing but safe for long distance, and only the rear wheels of the car would touch down behind.

The car only weights 1800 lbs so half that would be supported mostly (yes it will peak much higher with bounces and engine etc).. and the ambulance is way too light for the suspension in the back with all the equipment removed (stiff as hell).

In my head I'm imagining something like a fold down hitch cargo rack but way thicker and with the forks to either fit around the wheels or with a disk that has cutouts to fit onto the wheel bolts directly (and remove wheels).

The car is the most fuel efficient vehicle ever made in decent numbers so I'd rather not spoil that with some tow dolly wheels and get it up tight with most simple mechanism possible.

just to add, I would perhaps just include/carry ramps or a high regular jack to bring the front end of the little car high enough to get under the arms rather than get a complicated hydraulic / wench system if possible.

i'll also add - i'm not proposing to hook onto the foot support seen, but rather the thick metal framing underneath the vehicle they are attached to

The things I'd be concerned about:
1) how able is the rear suspension able to function when the car is tilted up? I've heard of suspensions being prematurely worn out due to the angle they were towed at.
2) how will you have enough pivot so the rear wheels aren't scrubbing during turns?

What chassis is the ambulance on and with the the up fit can it take ~1000-1500lbs of tough weight at the distance from the rear axle you could put a hitch. ( Scale the truck and calculate your rear axle and gvw according to the attached image) if your truck can handle the weight get a fabrication shop to engineer and build a rigid tow frame for the front of the car that picks up the front wheels then you just need lights and brakes if required by your local laws. If all of this works out it will tow like a normal if tongue heavy.

Edit you will also need a properly engineered hitch the step supports won't hold the load of a hitch
