Built on an AWD Astro chassis, has the vortec 4.2 V6, pop up top, fully self contained,

The Provan people were located in Colorado before their move to Georgia. So it's likely
that this van was originally purchased by someone in this area.

More or less, yes. There were a couple of different headlight/grill arrangements on the 2nd-gen Astros (and another arrangement entirely for the Safari), but that is a good example of the "GT" (pop-top) version (versus the XL hard-top version)

That's a 2003-2005 model. The 6 lug wheels is the tell. The 1999 will have 5 lug.
