Should I purchase an axle with toyota lug patterns, or should I put adapters on a standard hub axle?
Yes, get the proper hub. Save the expense and nuisance of extra parts and extra nut to torque
Won't I need to get additional legnth on the axle ends after the leaf perches to allow for a different offset vehicle wheel instead of skinny trailer wheels?

I THINK this is true.
Toyota rims are hub-centric. Trailer axles are lug-centric.

This may give you a headache but that's part of teardrop building.
I'm not sure how this will affect what you are trying to do by running Tacoma rims on a trailer axle. It's doable but this is a detail that needs some attention.
