Help with foam cut outs

It really depends on the type of foam you're using. Have you tried Kaizen? It's a multi-layered foam grade meaning you can cut through it with a sharp blade in increments. Other types I've had reasonable success with is this type of HLB packaging foam and Plastazote closed cell foam.
 
Serrated carving knife, longer the better. Shallow cutting angle, long draw, light pressure. If you just hack and saw you'll get a terrible ragged cut.
