I am not trying to find a 4 door ambulance 4x4....

I want to buy a truck like this http://sanantonio.craigslist.org/cto/4144192828.html Then mount the ambulance box on the back of it, then cut a hole in the back of the fords cab and connect the two with this http://www.gortite.com/bellows/bus-light-rail-bellows. The end result will be a 4x4,7.3L turbo diesel, 4 door, dually, with 4 captain chairs and no center console so that you can get to the back front either the front seats or the rear seats and have seating for 4. and 4x4 parts and lifts readily available.

Good idea! If you do it the earthroamer way (which is, when I get right from the ER pictures) a cabin fixed mounted to the box, you will get into trouble with the flex of the frame. An impression: http://www.hackneys.com/mitsu/index-pivot-frame.htm
I know a lot people who use brushes fixed to the cabin and something to hold the water aside at the box at the top. Works great at several bigger trucks here for years. Simple and nothing that can break.

Finally it depends on how much flex you will have which solution woks best. I´ve seen some torn Bus bellows torn. Meaning on/offroad use that cause flex. Fixed box or with silend blocks or with 3 or 4 point pivot mount. If you are going offroad - and guess you will if choose a 4x4- there is always a weakest point.

It is easily doable. The frame is the same with all F series. The box is connected to the cab with bellows, so they can flex independently of each other. You would want to change axle for a chassis cab axle. They are narrower. A normal dually axle will stick out on each side.

I would look for a mini mod box. Shorter in length, makes over all truck length shorter.
