Google the Jeep Lower Forty concept vehicle for one example.

Jeep engineers managed to fit 40 inch rubber under a 2door Wrangler mostly by modifying the sheet metal fenders. They used Dynatrak axles from the Mopar catalog, and changed the gearing in the differentials to account for the larger tire diameter. The Lower Forty also has a V8 conversion.

Haven, I think it was more than sheetmetal mods. I think they did some mods to frame and suspension to accomodate the tires, just no "lift".
 
I run 40s on my overland LJ and 37s on the 4 door JK. Took a 4500 mile trip across the north west last year in the LJ and got 14-15mpg with my RTT on the roof. I do have a 5.9l v8 and 4.88 gears but that's crusing 75+.
The JK will eventually get 40s but I need to finish off the kevlars first.
I love 40s they are the new 37 down this way. Although they are heavy so more power is needed.

Absolutely agree on the 40s usage. Very doable. I ran 3 years on my 40s; only recently downsized due to a great tire deal.

Supercharger now makes me rethink 40s ;)
