You totally inspired me. I just dropped my truck off at a metal fabricators here in Managua for the creation of a exo-skeleton rack like you have done. I even printed off photos from your posts to guide the lads in the shop...hope thats okay!
My personal mods will be it will have ladders on both sides right behind the second cabin doors, with lateral bars through the line of the windows for theft prevention (well there is no "prevention"....just an attempt to disuade...) & possible lashing points for surf boards/kayaks/ traction mats/ etc..and a safari basket on the roof with thule-like cross bars for kayaks. I am super excited for it...going to be an elevated sleeping nest, surf scouting platform and general crap store-er. All in, with custom sliders its going to cost me $400 USD installed. I LOVE it!
