We were just at the Southeast Overland Rally and Exp in Murphy, NC http://www.overlandrally.com/Overland_Rallies_and_Workshops/Southeast.html (posts #374 & #381) where we saw some VERY nice 4WD van conversions from U-Joint Offroad http://www.ujointoffroad.com/. They make a LOT of accessories and had some HUGE roof racks on display for their vans. The Sportsmobiles may be out of the price range for a lot of people, and this company seems to offer a good alternative and offers good quality kits and pieces if you wanted to "do it yourself." They are on the East coast too which may help geographically if you ever were to purchase one of these machines.
PS The ambulance conversion was awesome too. I believe the guy said he paid less than $4,000 for the base vehicle. This is an alternative for a donor vehicle that I wouldn't have considered before.
