It looks like here is still room to use a bumper tire mount instead of he door mount, or does the camper extend so a swing style bumper wouldn’t fit?

Agreed. Turbo diesel and 4WD in America... very rare indeed. There was one that sold for almost 6 figures before. It was in better shape but still. If anyone wants one they should act fast.

It’s seems expensive for what it is. Not a lot of interior storage and ok tankeage. For 110k without the truck I expected more. For that money you could have a custom XP cube, Uro Camper (import required), and many typical truck campers including a side entry Alaskan for a lot less.
Another thing to maybe learn from the experience when traveling in remote areas is to fill the water tanks and dump the grey and black (or cassette in your case). Then you would be ready to go for days.
We haven’t been that remote but with our Sprinter we dump the tanks before we leave in...
I agree with the Revel as a good choice, or maybe a used Sportsmobile? With your wife’s health issues I wouldn’t waste valuable time on a perfect custom van. If the Revel has most of what you want I would go for it.

I like cabover trucks and wondered if anyone built a camper or 4x4 conversion on a Ford LFC (e.g. Ford van or truck 4x4 components)? It looks like an interesting package.
I haven't driven one either, but we have a 144 Sprinter based RV and love that size and driving style. The cutaway Sprinters are not available in the US with 4x4 and the modern diesels are a problem in S. America, hence the Transit option. For our off road needs I think a van based vehicle...
