Depends on the use.
For this type of powerplant, I'd usually opt for a taller gearset to get out to the trail - there's still a crapton of usable torque from a 6BT, even at low rpm's, for trail use.
Also depends on what the split is for 1st gear, if that's a low sucker then I have no issues...
In the Suburban with the Cummins and NV4500, I was running about 14.5 mpg. 4.11 front and rear, D60 front, corporate 14 rear, detroit lockers in both, 35" tires.

So far, I've got 48k miles on ours, no duo issues, but other issues for sure, like scr and sensors. I'm still saying that if there's a major drivetrain failure, this truck will be getting a Cummins and Allison combo.
