I don't know the Chevys very well, but I personally wouldn't be running those mods without a tune to compensate. You could very well be running lean and the PCM can't figure out what to do.

14 US MPG in the city on my 2014 3.7 F150 on 34x10.5R17 with a topper and small lift.
Windy and hilly towing a 2500# pop up camper with mountain bikes on the roof rack of camper and 800# in the truck was about 13 US MPG.

A 4" lift is a cut/drop cross member lift. Why bother when you can fit 35s on a 2" proper strut lift? Your cross member will still be the same lowest point on the truck after a 4" lift.
