You could always get a custom made set of headers, then you could use whatever flanges you want and you would also have more flexibility with the turbo location.
Just a thought...

Another place you could touch base with is Turbo Glide here in Australia, as I know they have been installing turbos on these motors for a long time.
They might be able to give you some insight on a suitable manifold. I have a feint memory that they may have made their own version for the Canter.
 
@soaringpotato I was looking at a similar adapter but it looks like it is rotated 90° from where it is supposed to be. The long side of the flanges should be aligned but maybe it is just the image. The other problem is the offset, I tore everything apart today and it looks like there is only about 1" of clearance between the turbo and the tube for the cab mount.

I am still racking my brain on how @JDMBrian mounted this. This is his photo and its definitely a Holset turbo on what looks like a stock manifold. There has to be a manifold out there somewhere with a T3 flange. He said early in his post that he had a 4D35T but I cant find anything when I search for a manifold for that motor.

I'm like 90% sure his manifold came from Kinugawa or similar. They produce, or used to, a manifold that looks to have a T3 mount.

 
I just sent another email off to Kinugawa to see if maybe they had a manifold for any other models or something they may have made in the past with the correct mount.

I also was going through some photos on the Turbo Glide site and saw they just welded and adapter plate to the stock manifold. That might be my best option at this point.

They just got back to me and said they checked with the warehouse and manifold suppliers and that they don't have a manifold with that bolt pattern. Maybe they did in the past but it seems strange that they would offer a different bolt pattern when they sell a TD05 turbo in the kit.
 
The exhaust manifold is from Turbo-glide in NSW, has a T3 interface on it. They can provide with other interface sizes.
I was just digging through some old posts and found this comment about a Turbo-glide manifold with a T3 interface. I was able to get a response back from them last night and they said they have custom castings for these and just need a couple weeks to machine. Now I just have to hope they will ship to the US. Still waiting for a response on that.
